Intake Forms and Sessions:

Once we have a short chat on the phone, I will send you the links and information to fill out the intake forms. I need to know a few key details so we can start to individualize your experience from the beginning. You do not need to have new lab results to get started and often it is better to have our first session before asking your MD for lab tests in case there are specific questions I might have that can be answered by tests that your MD might not think to order.

If you do have recent lab results (within the last 12 months) and you can send them to me via fax or email, that will give me some great baseline information as we get started. To have these sent to me directly, please be sure to give your primary care doctor a completed and signed copy of the Medical Information Release form (which is also included in the intake form email) or you can click above to download it by itself. If you have not had lab work done within the last 6 months, I may request certain labs be performed to help build your program with more clear direction.

All appointments are virtual at this time. Video call platforms include Zoom, Skype, or FaceTime depending on your comfort level. If video calling is unavailable, phone is the next best option. In-person home visits may also be an option if desired and appropriate covid protocols are agreed to in advance. Home visits are offered within 10 miles of one of my locations during times when I am there (Southern Marin, CA; Klamath Falls; OR, Ashland, OR; Portland, OR; Orcas Island, WA; and Richmond, VA).

If you prefer to pay with a credit card, debit card, health savings account debit card, or flex plan debit card, I can accept them via Square, Stripe, or PayPal for your convenience. There is a nominal processing fee for this service.
